Heart Failure Management Program
Heart Failure is the nation's most rapidly growing cardiovascular
condition. It affects an estimated 5 million Americans. Patients
with heart failure typically require frequent office and emergency
room visits and hospital admissions. The management of this
disorders is complex, costly and challenging to providers
and patients alike.
The Heart Failure Management Program aims to improve quality
of life for hear failure patients by increasing their knowledge
about heart failure while decreasing hospital admissions.
The program helps to integrate inpatient and outpatient services
for heart failure patients.
Eligibility Criteria
Patients with heart failure are eligible to participate in
the program if they may benefit from additional evaluation
and education regarding the management of heart failure.
Referral Process
A physician referral is required. A cardiologist or primary
care provider may refer patients
Services Available
Registered nurse specialists coordinate the Heart failure
Management Program
Outpatient Services
Outpatient services are coordinated with the referring physician.
Services available include:
- Telephone Monitoring
- Clinical Monitoring
- Comprehensive Heart Failure Education
- Heart Failure Exercise Program
- Adjustment of Heart Failure Medications
- IV Medication Administration
Inpatient Services
· Patients admitted to the hospital for heart failure
are visited by the program's nurse specialists during their
hospital stay. Education about heart failure and its treatment
with medication is provided to patients and their families.
· The program's nurse specialists coordinate care with
other inpatient providers, such as dieticians and exercise
specialists, and offer assistance with discharge planning.
